Lead forms and landing pages must include clear, conspicuous consent language. Below is a full example that covers SMS, voice calls, and AI — suitable for insurance lead generation forms:
By checking this box, I provide my express written consent to be contacted by [Agency Name] and its authorized partners, including via automated text messages (SMS/MMS), automated and prerecorded voice calls, and artificial intelligence (AI)-generated or AI-assisted calls and messages, at the phone number provided above, for the purpose of providing insurance quotes and related information. I understand that:
- Consent is not required as a condition of purchasing any insurance product or service.
- Message and data rates may apply. Message frequency varies.
- I may revoke consent at any time by replying STOP to any text message or by contacting the agency directly.

Why this language matters:

We designed LeadsHello so that agents using the platform are protected by default. Our automated systems enforce the rules below without requiring manual action from agents.
Prior Express Written Consent
Under the TCPA, businesses must obtain prior express written consent before sending automated marketing text messages to consumers. This means:

- ✓ The opt-in must clearly disclose that the consumer agrees to receive automated text messages.
